Recovery timeline at a glance
| When | What you'll notice | What to do |
|---|---|---|
| Day 0 | Procedure day. Mild swelling possible. Tight feeling in donor area. | Sleep slightly elevated. Saline mist as instructed. Tylenol if needed. |
| Day 1–3 | Tiny scabs forming over each graft. Mild forehead swelling possible. | No bending over. No gym. No sun. Gentle saline only. |
| Day 4–7 | Scabs dry. Donor area feels nearly normal. Most return to desk work. | Begin gentle rinses per instructions. No swimming. No hats touching scalp. |
| Week 2 | Scabs gone. Hair shafts may shed — this is expected. | Resume normal washing. Light exercise OK. No heavy lifting. |
| Week 3–4 | Shock loss: most transplanted hairs shed. The follicles remain. | Don't panic. This is the normal cycle, not a failed transplant. |
| Month 2–3 | Scalp looks similar to pre-procedure. Early new growth begins. | Continue any prescribed medical therapy. Patience is the work. |
| Month 4–6 | Visible new hair, gradually thickening. Density still building. | Photograph monthly to track progress objectively. |
| Month 9–12 | Final density and texture. Style and treat as native hair. | Yearly check-in to plan any refinement session. |
Things to avoid in the first 14 days
Direct sun
Wear a loose hat after day 5 or stay in shade. UV can damage healing grafts.
Strenuous exercise
Sweat irritates grafts and increases swelling. Light walking is fine; resume gym at 2 weeks.
Swimming
Pools, hot tubs, and the ocean are off-limits for at least 14 days.
Alcohol
Avoid for 5 days post-op — it impairs healing and increases bleeding risk.
Smoking
Smoking reduces blood flow to grafts. Pause for at least 2 weeks; ideally longer.
Hats that touch grafts
Use only loose-fitting, non-touching headwear for the first week.
When to call us
You should reach out if you notice: increasing redness, warmth, or pus in the donor or recipient area; a fever over 100.4°F; persistent bleeding from the donor area beyond the first 24 hours; or unusual pain that's getting worse rather than better.
None of these are common, but they're easier to handle early. Local follow-up is one of the main reasons to have your procedure in Fresno rather than abroad.
Frequently asked questions
When can I go back to work?+
Most desk-job patients return at day 3–7. Anyone in physical work, dust, or sun-exposed environments should plan 10–14 days off.
Is shock loss permanent?+
No. The shedding at week 2–4 is the natural hair cycle resetting. The follicles are still in place and produce new hair starting around month 3.
When can I exercise again?+
Light walking at any time. Cardio at 2 weeks. Heavy lifting at 3–4 weeks.
When can I wear a hat?+
Loose-fitting, non-touching hats after day 7. Tight hats not until 3 weeks.
When will I see real results?+
Visible growth at 3 months, meaningful density at 6, final result at 9–12 months.
